Program Release Forms
In order to participate in TreeHouse trips and activities, a parent and/or guardian needs to complete one or more of the following release forms.
TreeHouse Activity Permission Slip– this is the main form completed for all general activities.
Release and Indemnification Form– this is the form used for all overnight trips, or high adventure trips such as rock climbing, canoeing, etc.
